- One in-person visit per month
- One tele-health or phone consult per month
- In-office lab draws (lab fees may apply)
- Ongoing medication management
- Unlimited additional visits for just $50 each, including urgent care needs
Prefer to pay per visit? No problem. We also offer:
- Initial well-visit: $120 flat fee (includes physical, lab draw if needed, med review, and full lab workup)
- Follow-up visits: $95 each
